Unless specified on a per-zone basis, there’s no level limit.
To achieve a particular rank for a zone, you must clear all the lower ranks for the zone, and then kill all the mobs for that rank.
Killing a mob means going in a zone alone, clearing a full spawn to the mob, killing the mob, looting it – ideally saving screenshots or even videos if the kill is particularly brag-worthy, and safely leaving the zone or the camp. Exceptions to this rule: getting a rogue open a locked door, …
